Notre premier programme \u00e9tait un centre de soins palliatifs cr\u00e9\u00e9 en partenariat avec la Mission d\u2019Ottawa, et depuis, nous avons \u00e9largi nos activit\u00e9s pour inclure des programmes en refuge, des logements avec services de soutien, des services de proximit\u00e9 et des initiatives men\u00e9es par des pairs. Notre travail vise \u00e0 offrir des soins \u00e9quitables et fond\u00e9s sur des donn\u00e9es probantes gr\u00e2ce \u00e0 une approche de r\u00e9duction des m\u00e9faits, en collaborant \u00e9troitement avec des partenaires de toute la ville. Aujourd\u2019hui, nous continuons de b\u00e2tir une communaut\u00e9 inclusive en am\u00e9liorant l\u2019acc\u00e8s aux soins, en r\u00e9duisant la stigmatisation et en soutenant les personnes ayant
