Environ 1,2 million de personnes qui consomment des drogues vivent avec le VIH, et 10 millions sont infect\u00e9es par le virus de l\u2019h\u00e9patite C (1). Le partage des aiguilles constitue l\u2019une des principales voies de transmission de ces infections (1). Jusqu\u2019\u00e0 r\u00e9cemment, l\u2019acc\u00e8s aux programmes d\u2019\u00e9change et de distribution de seringues n\u2019\u00e9tait pas aussi r\u00e9pandu. La fourniture de seringues st\u00e9riles est un \u00e9l\u00e9ment cl\u00e9 de la r\u00e9duction des m\u00e9faits, car elle contribue \u00e0 limiter le partage de mat\u00e9riel et \u00e0 r\u00e9duire les taux d\u2019infection (2). Les seringues peuvent \u00eatre \u00e0 embout court ou \u00e0 embout long, selon les pr\u00e9f\u00e9rences des personnes qui les utilisent.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Pour transformer certaines drogues, comme le crack, en solution injectable, un acidifiant est n\u00e9cessaire. La vitamine C constitue l\u2019option la plus s\u00fbre, tandis que des alternatives comme le jus de citron ou le vinaigre peuvent provoquer des infections et des l\u00e9sions veineuses (3). M\u00eame si la vitamine C est recommand\u00e9e, il est important de savoir que tout acidifiant peut endommager les veines (3). Le liquide injectable se pr\u00e9pare en m\u00e9langeant de l\u2019eau st\u00e9rile avec la drogue. Il est essentiel d\u2019utiliser de l\u2019eau st\u00e9rile plut\u00f4t que d\u2019autres sources (salive, eau du robinet, flaques d\u2019eau) afin de pr\u00e9venir toute infection (4). Il est \u00e9galement important de savoir que l\u2019usage d\u2019alcool pour pr\u00e9parer la solution peut augmenter le risque de surdose, surtout lorsqu\u2019il est combin\u00e9 \u00e0 des s\u00e9datifs comme le fentanyl (4). Pour cette raison, l\u2019utilisation d\u2019eau st\u00e9rile est fortement recommand\u00e9e lors de la pr\u00e9paration des substances \u00e0 injecter.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

R\u00e9cipients de chauffe<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Les r\u00e9cipients de chauffe m\u00e9talliques servent \u00e0 pr\u00e9parer la drogue avant l\u2019injection. On y met de l\u2019eau st\u00e9rile, la drogue et, parfois, de la vitamine C, puis on chauffe le m\u00e9lange \u00e0 l\u2019aide d\u2019une source de chaleur, comme un briquet. Pour pr\u00e9venir les infections, il est recommand\u00e9 d\u2019utiliser un r\u00e9cipient neuf \u00e0 chaque injection. Si le r\u00e9cipient doit \u00eatre r\u00e9utilis\u00e9, le m\u00e9lange doit \u00eatre chauff\u00e9 pendant au moins 10 secondes (5). Le partage de r\u00e9cipients de chauffe, m\u00eame sans \u00e9change d\u2019aiguilles, peut favoriser la transmission d\u2019infections. L\u2019utilisation de filtres peut aider \u00e0 r\u00e9duire plusieurs complications li\u00e9es \u00e0 l\u2019injection de drogues. Une fois la drogue pr\u00eate \u00e0 \u00eatre inject\u00e9e, il est important d\u2019utiliser un filtre pour aspirer le m\u00e9lange dans la seringue. Ces filtres permettent de diminuer le risque d\u2019injecter des impuret\u00e9s ou des particules \u00e9trang\u00e8res pouvant provoquer infections et blessures (6).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

La pipe \u00e0 tige droite est utilis\u00e9e pour l\u2019inhalation de diverses substances, le plus souvent du crack. Elle pr\u00e9sente moins de risques que les pipes en cuivre, en plastique ou les dispositifs de fortune faits de canettes de boisson, qui peuvent d\u00e9gager des \u00e9manations toxiques lorsqu\u2019ils sont chauff\u00e9s. Elle est \u00e9galement pr\u00e9f\u00e9rable aux pipes en verre fabriqu\u00e9es \u00e0 partir d\u2019objets impropres \u00e0 cet usage, tels que des seringues ou des ampoules, lesquelles sont plus susceptibles de se briser et de causer des blessures (7). Bien que les risques associ\u00e9s au partage des aiguilles soient largement reconnus, il est tout aussi important d\u2019\u00e9viter le partage des pipes, car cette pratique peut \u00e9galement favoriser la transmission d\u2019infections (8).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t

Les grilles en laiton se plient et s\u2019ins\u00e8rent dans les pipes pour maintenir la drogue en place et r\u00e9duire le risque d\u2019inhalation de fragments. Plusieurs personnes qui inhalent des drogues utilisent encore des produits commerciaux \u00e0 base de laine d\u2019acier, tels que les tampons Brillo, comme solution de remplacement (9). Or, ces produits peuvent comporter divers risques pour la sant\u00e9, notamment l\u2019inhalation de substances chimiques et de particules d\u2019acier (9). Par ailleurs, il a \u00e9t\u00e9 d\u00e9montr\u00e9 que les grilles en laiton retiennent davantage de drogue que la laine d\u2019acier, ce qui permet de r\u00e9duire les pertes (9).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Les tampons imbib\u00e9s d\u2019alcool jouent un r\u00f4le essentiel en mati\u00e8re de r\u00e9duction des m\u00e9faits. Ils permettent de d\u00e9sinfecter diff\u00e9rents objets avant la consommation de drogues, notamment les mains, les pipes, les seringues ainsi que les surfaces utilis\u00e9es pour l\u2019injection. Il est important de ne pas r\u00e9utiliser les tampons d\u2019alcool (7). Pour pr\u00e9venir infections et blessures, les seringues et surfaces d\u2019injection doivent \u00eatre nettoy\u00e9es avec des tampons imbib\u00e9s d\u2019alcool. L\u2019usage de salive pour nettoyer une aiguille ou une surface peut provoquer des abc\u00e8s ou des cellulites (4). Il ne faut jamais partager le mat\u00e9riel de consommation (pipes, seringues), m\u00eame apr\u00e8s d\u00e9sinfection avec un tampon imbib\u00e9 d\u2019alcool, et il faut \u00e9galement \u00e9viter de le r\u00e9utiliser autant que possible.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t
