Unit\u00e9 de soins sp\u00e9ciaux pour hommes<\/h4>

<\/i> Arm\u00e9e du Salut, 171, rue George <\/span><\/a>

\nSitu\u00e9e au troisi\u00e8me \u00e9tage du Centre Booth de l\u2019Arm\u00e9e du Salut, l\u2019unit\u00e9 de soins sp\u00e9ciaux pour hommes peut accueillir jusqu\u2019\u00e0 30 hommes ayant besoin d\u2019un soutien intensif en sant\u00e9 physique, en sant\u00e9 mentale et en d\u00e9pendance. En partenariat avec l\u2019Arm\u00e9e du Salut, l\u2019objectif est d\u2019accompagner des hommes pr\u00e9sentant des besoins complexes afin de contribuer \u00e0 la stabilisation r\u00e9sidentielle et \u00e0 l\u2019acc\u00e8s \u00e0 un logement offrant le niveau de soutien n\u00e9cessaire pour b\u00e2tir un avenir en sant\u00e9.<\/p><\/div><\/div>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Programme TED (intervention et soutien cibl\u00e9s)<\/h4>

<\/i> Bergers de l\u2019espoir, 256, avenue King Edward <\/span><\/a>

\nLe programme TED (intervention et soutien cibl\u00e9s) offre des soins de sant\u00e9 dans le cadre du programme TESP (Temporary Enhanced Shelter Program), g\u00e9r\u00e9 par les Bergers de l\u2019espoir. Il vise \u00e0 fournir des soins et des traitements accessibles aux personnes sans logement pr\u00e9sentant des besoins complexes en mati\u00e8re de sant\u00e9. Les services offerts comprennent des soins infirmiers, des services en sant\u00e9 mentale, une gestion intensive de cas, un accompagnement par les pairs pour les rendez vous, des services de pratique infirmi\u00e8re avanc\u00e9e, des services de psychiatrie, l\u2019acc\u00e8s \u00e0 un interniste ainsi qu\u2019un suivi m\u00e9dical continu. Le programme assure \u00e9galement une pr\u00e9sence de surveillance 24 heures sur 24 pour les personnes sous l\u2019influence de drogues ou d\u2019alcool, leur permettant de vivre un processus de sevrage s\u00e9curitaire dans la communaut\u00e9, plut\u00f4t que dans un service d\u2019urgence hospitalier.<\/p><\/div><\/div>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

<\/i> Mission d\u2019Ottawa, 53, avenue Daly<\/span><\/a>

\nAvec un centre de soins palliatifs aigus de 14 lits et un programme de soins palliatifs chroniques de 7 lits, l\u2019hospice Diane Morrison accueille, au cours des derni\u00e8res ann\u00e9es et des derniers mois de leur vie, de nombreuses personnes de la communaut\u00e9.
