(OICH) a vu le jour en 2001 pour venir en aide aux prestataires de soins de sant\u00e9 locaux qui peinaient \u00e0 r\u00e9pondre aux besoins des personnes sans domicile fixe pr\u00e9sentant des probl\u00e8mes de sant\u00e9 complexes. De nombreux membres de notre communaut\u00e9 mouraient du VIH\/sida dans des refuges pour sans-abri, dans la rue, chez des amis ou \u00e0 l\u2019h\u00f4pital. Nous avons lanc\u00e9 notre premier programme : un centre de soins palliatifs en partenariat avec l\u2019Ottawa Mission.<\/p><p>Depuis lors, nous avons continu\u00e9 \u00e0 nous d\u00e9velopper et \u00e0 ouvrir d\u2019autres programmes dans les refuges pour sans-abri de la ville, notre objectif \u00e9tant de garantir que les personnes confront\u00e9es \u00e0 des obstacles \u00e0 une bonne sant\u00e9 b\u00e9n\u00e9ficient de la m\u00eame qualit\u00e9 de soins que les autres Canadiens. Nous y sommes parvenus gr\u00e2ce \u00e0 une meilleure coordination et int\u00e9gration des services destin\u00e9s aux personnes sans domicile fixe souffrant de troubles mentaux chroniques et de probl\u00e8mes li\u00e9s \u00e0 la consommation de substances.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-50 elementor-inner-column elementor-element elementor-element-96f0001 elementor-invisible\" data-id=\"96f0001\" data-element_type=\"column\" data-e-type=\"column\" data-settings=\"{&quot;animation&quot;:&quot;fadeIn&quot;}\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-c1f4a56 elementor-widget elementor-widget-text-editor\" data-id=\"c1f4a56\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Tous les programmes de l&#8217;OICH sont mis en \u0153uvre en partenariat avec des centres d&#8217;h\u00e9bergement, des organismes de logement, des h\u00f4pitaux et des prestataires de soins de sant\u00e9 communautaires.<\/p><p>Aujourd&#8217;hui, plus de 20 ans plus tard, l&#8217;OICH g\u00e8re 4 programmes en centre d&#8217;h\u00e9bergement et 6 programmes de logement accompagn\u00e9, ainsi que des services de proximit\u00e9 et des services de consommation supervis\u00e9e et de traitement destin\u00e9s \u00e0 aider les personnes confront\u00e9es \u00e0 des probl\u00e8mes de sant\u00e9.<\/p><p>Les soins de sant\u00e9 que nous dispensons s&#8217;appuient sur des recherches fond\u00e9es sur des donn\u00e9es probantes, un cadre de r\u00e9duction des risques et un amour profond pour nos clients et l&#8217;incroyable communaut\u00e9 qu&#8217;ils nous apportent. Nos clients sont r\u00e9silients et forts, et ils continuent d&#8217;\u00eatre nos plus grands enseignants, jour apr\u00e8s
