Nurse Practitioner

The role of the nurse practitioner is to manage the health care of clients in the Inner City Health.

The role of the nurse practitioner includes:  engaging the client in a therapeutic relationship, assessing the client’s needs, referring the client for admission, assisting the client to complete the admissions process, organizing a suitable plan of care, coordinating care activities, monitoring compliance to the care plan, providing the full range of primary health care consistent with the NP scope of practice.

The nurse practitioner is an advanced practice nurse with expertise in health care of the homeless.  The nurse practitioner provides leadership to an interdisciplinary team of health and social services providers and coordinates their efforts to meet the client’s needs. Qualifications include: Meets the standards of practice of the College of Nurses of Ontario. Possess a current Registered Nurse (EC), Certificate of Competence.
